Job Description
MVRA is seeking a Mental Health Case Manager to join our Mental Health Department, supporting adults receiving Section 17 Community Integration services. This role uses a strength-based, solution-focused, trauma-informed approach to support individuals in identifying needs, barriers, and strengths.
Key Responsibilities
- Meet one-on-one with individuals eligible for Section 17 services
- Complete required documentation, including assessments, LOCUS scoring, AC-OK screenings, and Individual Treatment Plans (every 90 days)
- Maintain weekly case notes and complete quarterly screenings related to employment, housing, and readiness for change
- Advocate for individuals by coordinating with clinical providers and community supports to ensure continuity of care
- Participate in group clinical supervision and individual supervision with the Program Manager
