Pharmacist

POSITION SUMMARY
The role of this position is to assist the Pharmacy Director in the daily operations, management of staff, evaluation, optimization and promotion of pharmacy services designed to provide value from improved patient outcomes and financial return through available revenue channels. Organizes and directs the workflow of operations within the department according to Katahdin Valley Health Center (KVHC) and Pharmacy policies, procedures, mission, goals and values. Provides direction for technician staff regarding medication management processes, including financial stewardship. Implements optimal systems of care in coordination with the Pharmacy Director, solves problems and facilitates processes required to achieve health system strategic objectives within budgetary targets. Additional responsibilities include management of assigned staff, quality measurement and process improvement, practice standards within area of responsibility and regulatory compliance.
RESPONSIBILITIES
Supervision
1. Provides direct supervision and direction of Pharmacy Technicians.
2. Provides clear oversight and direction for the Pharmacy Technicians in such a way that all professional and technical activities performed are consistent with the KVHC mission, core values, supporting behaviors and policies.
Operations Management
1. Fills prescriptions in an accurate and timely manner. Reviews and verifies or enters orders into the medication management information system. Assures that orders present no significant problems in regard to drug interactions, incompatibilities, therapeutic duplications or patient safety.
2. Supervises and directs work of pharmacy technicians. Coordinates medication preparation and distribution work of the pharmacy technicians and ensures accuracy prior to dispensing medications for distribution.
3. Provides counseling to patients, families, or caregivers regarding their medications, to support appropriate and effective drug use. Provides accurate, appropriate and timely information to physicians, nurses, patients and others regarding medication therapy, drug product guidelines, law and regulations, medication formulary and other topics related to medication use and control.
4. Supervises the day-to-day operations of the pharmacy that include inventory management, interactions with insurance companies, and accurate and appropriate record keeping.
5. Consults with prescribing practitioners to help ensure accurate and appropriate medication use. Reviews medication orders and evaluates their appropriateness and completeness. Contacts theprescriber with suggestions for improvement and resolves problems with the medication orders as needed. 6. Monitors daily activities as they relate to the procurement, storage, preparation, distribution, compounding, monitoring, control and dispensing of medications and supplies. Ensures that there is a mechanism for accountability for these tasks. Evaluates existing service levels, resources and staffing and makes recommendations to the Pharmacy Director to ensure that service needs of all customers are met.
7. Manages the daily operations of the pharmacy as it relates to staffing, medication procurement, indigent care, dispensing, and patient counseling, coordination of selected reimbursement scenarios and determination of patient eligibility for service.
8. Manages the non-sterile product preparation process and ensures that adequate controls and standards are maintained in order to safeguard the patient, care provider and employee.
9. Actively supervises controlled substance procurement, documentation, storage, distribution, disposition and usage audits.
10. Comply with the 340B program requirements and all related operational elements to ensure program integrity and optimization in coordination with the Director of Development / Grants Management and Pharmacy Director.
Clinical Roles
1. Participates in pharmacy services through collaboration with physicians and other health care providers in the management of medication therapy in the ambulatory care patient population, including direct patient care pharmacotherapy management, provision of drug information and practice support.
2. Participates in physician, team and staff education and training for integrated care; continuing education about integration and evidence-based practice is routinely completed to maintain knowledge and skills.
3. Provide Medication Therapy Management (MTM) services and associated clinical pharmacy services to optimize drug therapy and improve therapeutic outcomes for KVHC patients by identifying, preventing and resolving medication-related problems.
4. Administer immunizations per 32 MRSA §13831-13835, according to and in compliance with the Pharmacy Rule Chapter 4-A, as authorized by KVHC Standing Order Protocol
5. Work as part of the Clinical Care Team to ensure effective and quality care is given to the KVHC patient.
Compliance
Assists in the development, implementation, interpretation and revision of policies and procedures. Ensures compliance with all applicable state, federal, AAAHC, URAC, OSHA, HCFA and HIPPA rules, standards, regulations and laws as applicable. Obtains and maintains specific certifications as determined by business needs (i.e. DMEPOS), execute and manage a compliant 340B Drug Program in accordance with Health Resources and Services Administration (HRSA) and Office of Pharmacy Affairs (OPA) regulations.
Outreach & Policy Development
1. Serves as a liaison with representatives doing business with KVHC. These activities include but are not limited to: product procurement, product recall, drug information, education on policies and procedures related to representatives activities within KVHC, contracts, distributionarrangements, customer service, and procurement systems, reverse distribution, physical inventory valuation and outsourcing arrangements, as pertains to medication management.
2. Promotes the practice of pharmacy through community or academic based activities including education, mentoring, active intervention and/or screenings.
3. Involvement in Pharmacy Policies and Procedures and Quality Initiatives.
Data Management
Demonstrates proficiency in the use of various healthcare and departmental information systems including:
1. Report generation and trending of various medication management data
2. Evaluation of new drug features and functionality as it relates to pharmacy information systems and technology that impacts the activities and services associated with pharmacy operations.
3. Evaluation of information systems and technology designed to improve pharmacy operations and patient safety.
4. Manages the collection of data from the quality control process and formulates recommendations on achieving compliance with practice standards or identification of performance improvement strategies.
5. Participates in the data collection and analysis of the KVHC programs.
Professional Development

8. Actively participates as committee member as assigned by the Pharmacy Director.

Contributes to the partnership among care givers, patients and families to provide safe and effective care.

EDUCATION AND EXPERIENCE
1. Eligible for pharmacist licensure in the State of Maine.
2. Advance degree preferred: PharmD. Completion of ASHP accredited Pharmacy Residency –preferred.
3. Previous experience in care pharmacy operations (i.e. retail, specialty) preferred.
4. Certification to administer drugs and vaccinations.
5. Demonstrated ability to establish and maintain effective working relationships with other members of the health care team, health system, educational and community leaders.
6. Evidence of excellent communication skills, verbal and written.
7. Experience using evidence-based decisions about drug use and patient safety.
8. Will be On-Call as needed.
9. Maintains all current credentialing requirements.
10. Must possess a clean criminal and DHHS background.
11. Must possess the ability and means to travel between clinics as needed.
12. Valid State of Maine Driver’s license and ability to be insured by KVHC’s vehicle insurance carrier.

Requires full range of body motion with the ability to bend, kneel and stoop.
