Job Description
The Case Manager’s primary focus is to address client’s needs through a
strengths-based approach. The Case Manager works in coordination with
partnered agencies to address the broad array of client needs through referral
to providers of necessary services, and in helping with reaching identified
goals. The Case Manager acts primarily as a care coordinator and client case
manager, and may perform duties that include:
- Outreach
- Prevention and risk reduction
- Crisis intervention management and referrals Program Implementation
- Case managers will assist clients in identifying needs and goals that are to be addressed. This will be fine through:
- Intake paperwork.
Case managers will help affect change for clients by:
- Assisting clients with referrals.
- Supporting clients with meeting their goals.
- Providing lists of providers and community resources to assist clients in obtaining services that are appropriate and helpful for the client.
- Providing outreach to community agencies and programs.
- Coordinating client care across programs and agencies.
- Focusing on strengths.
- Advocating for clients.
- Identifying areas of need and weaknesses and providing resources for those needs.
- Helping clients identify informal supports and resources.
- Client assessments.
- Questionnaires.
- Facilitating discussions with clients, their supports, providers, and community agencies.
- Creation of a treatment plan.
