Job Overview: We are seeking a highly skilled and compassionate Nurse Practitioner to join our dynamic team as a Long-Term Care Nurse Practitioner in a per diem role. Responsibilities:- Conduct comprehensive assessments of patients in long-term care facilities, including medical history, physical examinations, and medication reviews.
- Develop and implement individualized care plans to address the unique needs of long-term care residents.
- Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care.
- Provide education and support to patients, families, and caregivers on disease management, preventive care, and overall health and wellness.
- Monitor and manage chronic conditions, collaborating with physicians and specialists as needed.
- Participate in regular team meetings and contribute to the development and improvement of patient care processes.
- Stay abreast of current healthcare trends, regulations, and evidence-based practices related to long-term care.
