Summary
Position Summary
The Patient Navigator supports safe patient and family centered care by meeting the specialized needs of patients in the assigned service line as they progress through a wide variety of health care settings. The position is responsible for facilitating care and surgical services to enhance quality, cost-effective clinical outcomes, to both meet patients' healthcare needs while also decreasing fragmentation and duplication of care.
The Patient Navigator works under the daily and clinical direction of the surgeons and providers within the practice, working collaboratively with providers and clinicians across the health care system to access resources, identify barriers, and to problem solve clinical and/or surgical access issues related to care-plan development and/or implementation. The position may also interact with external providers and agencies to identify and coordinate appropriate services.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education: Bachelor's degree in Nursing. Master's degree preferred.
Licensure/Certifications: Current license to practice as a Registered Nurse in the State of Maine.
Experience: Experience working with patients within the service line preferred, in inpatient, outpatient or community-based care settings.
Evidence of ongoing education to maintain proficiency appropriate to the service line
Knowledge of community resources
Exceptional communication, interpersonal and organizational skills, to be able to function effectively in a complex health care environment and with patients and families from all backgrounds.
Computer proficiency
