Job Description

Join a dedicated and exciting team, working in a progressive, supportive, home health care environment. Assessment, planning, and provision of appropriate services as part of total care plan. Physical Therapist will provide professional physical therapy services in the patient’s home, school, or residential facility in accordance with physician’s orders and agency policies and procedures. Provides consultant physical therapy services when appropriate.
