Position Summary
Community Health Worker:
We have an exciting opportunity for an experienced Community Health Worker to join our growing Population Health Management (PHM) team! The PHM department provides care coordination services to patients across the MaineHealth Medical Group (MHMG) and is embedded within the primary care setting with an occasional home visit or visit in the community setting. As part of an interdisciplinary department, the CHW will work directly with patients to identify and address social determinants of health (SDOH) related barriers to care such as food or housing insecurity, transportation, care affordability and health literacy. The role is not expected to perform hands-on or direct patient care, nor are they expected to transport patients in a vehicle.
We are looking for someone who enjoys helping others, being part of an evolving team, helping to integrate clinical programs and community resources into the practice workflows with the intent to optimize patient care, improve patient and provider experience and enhance the quality of care provided to our patients.
Required Minimum Knowledge, Skills, and Abilities (KSA's)
Education: High school diploma or equivalent.
License/Certifications: Candidates must possess a valid Maine driver's license, maintain a clean driving record and have a personal vehicle available for work use.
Experience: Must be bilingual in English and any of the following languages: Arabic, Khmer, Portuguese, Somali, Spanish, Vietnamese or other specific to the communities being served.
Must be a resident of the community being served with good knowledge of the resources of their community.
Experience in outreach, customer service, social service, or related field preferred.
Experience in interpretation and translation is required.
Medical interpreting training and experience in a medical setting is preferred.
Excellent oral and written communication skills.
Fluency (verbal and written) in specified language.
Ability to carry out written and oral instructions.
Ability to exercise judgement in the application of professional services.
Comfortable with home visits and community or targeted outreach to patients who share a common background (life experience, socioeconomic status) with the candidate.
Comfortable with Microsoft Office suite.
