Uplift, Inc is a private nonprofit corporation. It exists to create conditions under which people with Intellectual and developmental disabilities experience presence and participation in a community life, respect and dignity, opportunities to make choices in the pursuit of a personal future, opportunities for developing and exercising competence, and opportunities to both create and maintain relationships. The corporation is governed by a Board of Directors, which is elected annually by the membership.

The Director of Case Management Services role will carry a case load in addition to providing overall case management support, direction, and coordination for adults with developmental disabilities. Statements included in this job description are an overview of the essential functions of this position. This position performs all duties within a team, which may allow for specific accommodations when needed.

Highlighted Responsibilities include but are not limited to:

  • Provide personnel management, including overseeing the operation, direction, and evaluation of all staff in a timely manner
  • Provide personnel case management services for adults with developmental disabilities, to include developing an individual support plan (ISP) and assisting the individual with implementing that plan
  • Coordinate all necessary services for the individual as outlined in the Person Centered Plan (PCP) to include but not limited to medical, dental, mental health, housing, finances, personal, social, and emotional supports, community integration, and all other services.
  • Assures compliance with applicable federal and state regulations and laws
  • Encourages and maintains cooperative working relationships with other agencies, families, and involved parties
