Adult Case Manager (ACM)

 

 

Assistance Plus has an immediate opening for an Adult Case Manager. The ACM will assist our clients with a Mental Health diagnosis that reside in the community by assisting in the development of community resources, coping skills and natural supports. The ACM is responsible for identifying unmet needs and to resolve barriers to needed services and will monitor the client’s supports to ensure that they match their individuals' needs.

 

******$1500.00 BONUS!******

 

Requirements:

  • MHRT/C or MHRT/CP certification
  • Employees must hold a valid Maine Driver's license
  • Have reliable transportation, and $50K/$100K automobile liability insurance
  • Must have a high school diploma or equivalent 
  • Current immunization records (preferred)
  • Intermediate level computer experience
