Job DescriptionDescription of Position:
The Child and Family Crisis Support Specialist will provide support to youth and their families through the expansion of aftercare and crisis stabilization services in the home setting. They will work to keep children in the least-restrictive environment by providing interventions to prevent the need for a higher level of care. This could include supporting children and their families in their homes while waiting for residential care, stepping down from emergency departments, crisis units, and inpatient units with the goal of returning to pre-crisis functioning and not returning to the emergency department or a higher level of care. This is inclusive of working with children who have mental health needs and/or co-occurring substance use, intellectual, and developmental disordersResponsibilities:
Engage families and children who are transitioning between levels of care (home, crisis unit, inpatient unit, residential). This would include families and children who have recently been involved in crisis services or need support as they step-down from a higher level of care to home.
Complete a safety and needs assessment and develop a client-centered plan of care that encompasses the child and family’s emotional, behavioral, and physical health needs.
Provide and coordinate referrals to all therapeutic interventions possible to assist the family and child to remain in the home and reduce the use of the Emergency Department and Inpatient care for up to 45 days post Mobile Crisis intervention. Interventions to include:
- crisis stabilization,
- safety and behavior planning,
- enhancing the skills and abilities of the child and family,
- family education about the child’s needs,
- referring for needed services and supports,
- supporting children to attend appointments,
- ongoing assessment of needs and supporting the family in addressing these needs as a means to support the child remaining in-home.
Remain involved when the child is waiting for a higher level of care and once a higher level of care is obtained; continue to follow the child by actively participating in discharge and aftercare planning for the child to return home. This includes supporting the child and family in the emergency department and being a liaison between services
Develop strong relationships with community partners, including but not limited to:
- Children’s behavioral health providers
- Primary care providers
- Law enforcement
- Emergency departments
- Children’s crisis stabilization units
- Children’s psychiatric inpatient units
- Residential providers
Bring stakeholders together to support the healthy development of children in our communities, address the needs, and exploration of other ways to support children and families getting their needs met outside of the emergency department.
Provide case consultation and coordination of services as required and consistent with agency Policy and Procedure and licensing requirements.
Partner with the Emergency Services Program and align as a resource in identified cases involving children needing higher levels of care. This may also include answering calls received through Mobile Dispatch, respond in person to requests for Emergency Services assessments if needed, and participating in the Critical Incident Stress Debriefing program services as offered by AMHC Emergency Services.

Provide integrated services in a manner that supports a client’s overall health and well-beingRequirements:
- Strong communication and collaboration skills, both verbal and in writing.
- Flexibility around offering services to meet community needs.
- Ability to multi-task, prioritize workload expectation, and problem solve.
- Must have a valid Driver’s license with available and reliable transportation.
This position requires a minimum of a Bachelor’s Degree in Behavioral Science, Psychology, Social Work, or other human services-related field from an accredited college or university, plus one year of direct relevant work experience. MHRT-CSP certification and additional de-escalation training is required to carry out the assigned responsibilities of the position. These will be provided by the agency upon hire
