Candidates must have excellent time management and customer service skills. Prior case management experience is preferred. Social Work supervision is provided, if needed!
The Community Case Manager coordinates the delivery of services for adults with intellectual disabilities who may also have physical disabilities. This includes a comprehensive assessment of individuals including identified support system, development of service plans, assisting the consumers and their families to link with the identified services, available service provider(s), regular monitoring of service plans and ongoing reassessment of consumers.
This is a full time 40 hours per week, non-exempt position that will be based out of the Spectrum Generations Home Office in Augusta, ME. There is also a potential for this to be a remote position from within the state of Maine.
A $1000 sign on bonus is being offered. $500 payable after completion of the 90 days and $500 after completing 1 year of service.
