The Certified Nursing Assistant is a nursing team member who provides assigned residents with routine daily nursing care and services in accordance with the residents’ assessment and Plan of Care, and as may be directed by supervisor.Job Related Responsibilities and Activities
+ Demonstrates the correct and safe use of all adaptive equipment to residents, according to the Plan of Care (POC) and as directed.
+ Provides and assists residents with adequate nutrition and hydration according to the POC and as directed.
+ Performs Activities of Daily Living (ADLs) for residents according to the POC and as directed.
+ Provides residents with restorative and rehabilitative nursing procedures according to the POC and as directed.
+ Accurately obtains Vital Signs (VS) and weights of residents as well as intake and output records (I&O), and calculates % of meal taken.
+ Monitors and reports on all residents’ skin condition for the presence of pressure areas and skin breakdowns to prevent decubitus.
+ Assures that all residents’ incontinence is treated in a dignified, appropriate and prescribed manner according to the POC and as directed.
+ Utilizes physical restraints only as directed by Charge Nurse, checks resident regularly and releases according to the POC and as directed.
+ Promptly reports all changes in the residents’ condition, complaints of pain/discomfort, complaints of grievances, injuries of any kind, missing residents, or unusual observations to the Charge Nurse as soon as practical.
+ Assures that all residents’ proper bowel and bladder function is maintained and monitored.
+ Follows approaches related to behavioral, spiritual, psychosocial, and personal needs of residents according to the POC and as directed.
+ Treats all residents fairly, with kindness, dignity and respect. Upholds residents’ rights.
+ Promptly and appropriately responds to residents and family requests and needs in a kind, understanding and respectful manner. Speaks to residents in a pleasant, helpful manner.
+ Assures that all residents have the opportunity to attend scheduled activities; assist residents in preparing for activity/social programs, and assists in transporting to programs.
+ Provides patient privacy by knocking on doors and greeting residents prior to entering rooms. Pulling curtain or closing door when rendering personal care.
+ Maintains a clean, comfortable, calm and safe environment for the residents.
+ Demonstrates a positive caring attitude as evidenced by relaxed, pleasant facial expression and gentleness during care of residents. Works diligently and focuses on maintaining harmony among team members and encourages teamwork.
+ Completes all assigned cleaning duties. Disposes of soiled clothing, linen, and trash according to MVH procedures. Makes beds, changes bed linens, and adds covers as requested.
+ Understands and implements the principles of infection control, consistently using standard precautions. Utilizes appropriate personal protective equipment, devices and equipment to accomplish tasks and protect self and others.
+ Completes accurate documentation on a daily basis regarding all care given to residents, using appropriate forms and abbreviations accepted by MVH, in an accurate, legible and timely manner.
+ Participates in receiving and giving change of shift report. Makes rounds and knows condition and location of residents on assignment.
+ Communicates professional needs, learning needs, and other needs to unit Charge Nurse and seeks clarification when needed.
+ Reports all work related problems, violations of resident’s rights, unusual events, unethical behavior, criminal activity, complaints/grievances, or other work-related concerns to supervisor right away and before the end of shift.
+ Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., assuring correct and comfortable positions. Performs duties in a manner that assures maximum safety to self and others. Operates equipment following manufacturer’s procedures and uses proper body mechanics.
+ Follows all facility, State, Federal and VA rules and regulations and MVH policies and procedures.
+ Assists residents to walk with or without self-help devices as instructed and to promote and maintain highest functional status.
+ Keeps the call-bell within easy reach of the resident. Answers resident call-bells promptly. Ensures that residents who are unable to call for help are checked on frequently. Checks each resident routinely to ensure that his/her personal care needs are being met according to his/her wishes.
+ Is familiar with POC for each resident on primary assignment.
+ Revises priorities when work changes so that the most effective job is done.
+ Demonstrates accuracy in performing skills/procedures as outlined in this job description and required by the role.
+ Attends and participates in staff meetings, annual in-service trainings and performance improvement activities.
+ Participates in the orientation of new staff to the unit as directed. Is helpful to new staff and others as needed.
+ Assists with the care of dying resident as instructed.
+ Provides post-mortem care as instructed.
+ Willingly accepts assignments. Approaches change and difficult situations in a professional manner. Promptly and cooperatively floats to other units as needed and directed.
+ Performs other reasonable related duties as assigned by the Charge Nurse.
Must have a Certified Nursing Assistant Certificate and be listed as an active CNA on the State of Maine Registry of Certified Nursing Assistants. Experience in Long-Term Care desirable.
