At the Maine-Dartmouth Family Medicine Residency Geriatric Medicine Program you will find a close-knit community of providers who are passionate about and committed to advancing best practice in the care of older adults across the continuum of care. Our group is comprised of two Nurse Practitioners and five Geriatricians who, in collaboration with a robust interprofessional team, provide care to older adults in a variety of settings which include an outpatient primary care hospital setting.
We have an opening for a certified and Maine-licensed Nurse Practitioner with excellent clinical skills, a passion for teaching and leadership development, and enthusiasm for interdisciplinary team-based care to join our team primarily working in the hospital setting. The ideal candidate should be knowledgeable about the unique care needs of older adults, including delirium prevention and management, polypharmacy, fall prevention, and supporting safe transitions across care settings. Also, given the unique role of our comprehensive and integrated program in optimizing and disseminating best practices to care for older adults across the health system, flexibility and willingness to adapt care models is important.
