Community Health Worker:
+ The Community Health Worker is a customer service professional that focuses on navigating the patient through their COVID-19 isolation. The navigator accomplishes this by collecting and documenting information from the patient and providing the necessary information for successful isolation.
+ Responsible for the following:
+ Receiving reports of persons confirmed as having COVID-19.
+ Interviewing identified people to determine if criteria has been met to be released from isolation.
+ Provide release of isolation documents via email or mail and update system database
+ Clearly and concisely gather information about symptoms, exposures and identifying others with whom might possibly be infectious.
+ Monitor and document the person’s compliance with isolation protocols
+ Enter information in a statewide database with the assurance of accuracy and detail.
+ Monitor incoming voicemail messages and direct to appropriate investigator.
+ Assist with updating database with investigator name and case status
+ Review reports to determine timely contact is made with each individual
+ Minimum Requirements:
+ Ability to manage, compile, analyze and present data.
+ Ability to communicate clearly, both orally and in writing with diverse audiences.
+ Ability to work with a variety of computer software packages.
+ Bachelor’s degree
