Job Description Summary: Come join our highly skilled Rehab Team at Graybirch Skilled Nursing Home and Long term Care. This facility has a 38 bed skilled program and also offers Therapy services to our long term care residents as well as residents in our Res-Care unit. Our Goal at MaineGeneral LTC is to provide the best patient centered care with a coordinated multidisciplinary approach. This per diem position will also provide opportunities to work on our hospital based 12 bed Inpatient Rehabilitation Facility and Acute Care at the Alfond Center for Health. Apply Today!
Assesses patient’s ability to safely and effectively perform speech/language skills. Designs the appropriateplan of care, including type and frequency of treatment.Implements the treatment plan using appropriate modalities. Documents subjective, objective, assessmentfindings and plan, and patient progress.Educates and counsels patients and family/caregiver.Plans and/or assists with patient discharge fromtherapy services and treatment setting.Provides information regarding appropriate selection/use of adaptiveequipment and support programs.
The ideal candidate will have:
Maine State Speech-Language Pathologist licensure requiredCertificate of Clinical Competence (CCC) by American Speech-Language-Hearing Association (ASHA) required within 1 year of hire.
