The Care Manager partners with, assesses, and plans interventions to help patients and/or families cope with social, emotional, economic, and environmental problems.
Participates in long term and/or discharge planning.
Gathers information through interview and assessment of patient and/or patient’s family, caregivers, and/or legal representatives. Provides and/or arranges for needed internal and external services, interventions or assessments as needed.
Consults and communicates with multidisciplinary teams regularly to evaluate patient’s status and coordination of care.
The ideal candidate will have:
Demonstrated knowledge of long-term care rules/regulations.
Knowledge and experience with Medicare, MaineCare and other reimbursement sources.
Bachelor’s degree in Social Work or Nursing preferred.
Experience in a skilled nursing/long term care environment.
Excellent organizational skills and ability to prioritize work.
Demonstrated ability in managing crucial or difficult conversations.
Maine state licensure as Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), or Registered Nurse (RN) required. Conditional Licensed Social Worker licensure may be considered for the right individual. Certification as a Case Manager (CCM) by the Commission for Case Manager Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association (ACMA) preferred.
