Position Highlights:The RN Case Manager will provide psychiatric nursing, community support and case management services to mental health clients with severe and persistent mental illness. In this role the candidate will function as part of a multidisciplinary, comprehensive mental health team in providing mental health services in the clients homes or environments. Employees assigned to this position are required to provide clinical on-call coverage on a routine, rotating basis.
Administers psychotropic medications per physician order and program policy.
In conjunction with the Rehabilitation and Recovery Nurse I, takes the lead in development of medication administration and monitoring for ACT Team clients who need this service.
Takes verbal and written medication orders from psychiatrist/physician and ensures accuracy of transcribing these orders into the medication record.
Monitors medication effectiveness and assesses for side effects of medication.
Assesses and monitors medication compliance.
Completes clinical documentation, utilization and continued stay reviews, and other reports as required by program administration and management.
Conducts comprehensive psychosocial nursing assessments.
Works with clients and ACT Team colleagues in developing treatment plans.
Establishes a positive therapeutic relationship. Communicates with ACT Team staff and works with treatment team to provide integrated services.
The ideal candidate will have:
Must have either a current State of Maine Registered Nurse (RN) license or an RN license recognized by the Nurse Licensure Compact (https://www.ncsbn.org/nlc.htm). If RN licensure is from a compact state, must obtain a state of Maine RN license within 90 days of changing primary residency to Maine.
Driver’s license required.
Associate’s degree, graduate of accredited school of nursing.
2 years or more of experience required.
