The Outpatient Care Manager is responsible for managing complex patient issues in relationship to needed services and the patient’s transition through the healthcare system. As a patient advocate, this person works collaboratively with referred patients and families from the five Patient Centered Medical Home (PCMH) pilot sites with an inter-disciplinary team both internal and external to the organization.


Formulates and implements a plan that addresses the patient’s identified needs and issues by assessing the patient/family needs, issues, resources and care goals; determining the choices available to individual patients; and educating the patient/family on the choices available.

Assists the patient to identify and/or develop their support system and encourages the patient to utilize their support system.

Identifies patients/families with significant health education needs, identifies the specific resources to meet those needs, makes the referral(s) and evaluates outcomes.

Triage and apply a collaborative team approach in identifying, analyzing, and resolving patient care problems.

Demonstrate ability to coordinate all levels of services and disciplines.

Determine clinical education needs of patients and their families and provide age and population specific care and education.

The ideal candidate will have:

Bachelor’s degree, BSN preferred.

Associate’s degree, Nursing required.

Must have either a current State of Maine Registered Nurse (RN) license or an RN license recognized by the Nurse Licensure Compact ( If RN licensure is from a compact state, must obtain a state of Maine RN license within 90 days of changing primary residency to Maine required.

3 years or more preferred.
