The Care Manager investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Develops a treatment plan; arranges for internal and external services/interventions.
Conduct a Psychosocial Assessment with patients and/or patient’s family caregivers.
Collaborates with multidisciplinary teams regularly to evaluate patient’s status and level of care, including coordination of care.
Participates in discharge planning including identification and coordination of appropriate resources. Provides discharge education and resource referrals to patients and families.
The ideal candidate will have:
Bachelor’s or Master’s Degree in Social Work or Nursing preferred.
Maine state licensure as Licensed Social Worker (LSW), LMSW, Licensed Clinical Social Worker (LCSW), or Registered Nurse (RN) required. Certification as a Case Manager (CCM) by the Commission for Case Manager Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association (ACMA) preferred.
