Adult Case Manager
Case Managers are professionals who play a key role in assisting adults with intellectual disabilities or autism to access resources needed to live a full and inclusive life. Case Managers provide thorough assessments of strengths and needs, comprehensive planning of service needs, advocacy, outreach and resource coordination unique to every person we serve.
Bachelor’s degree in Human Services, Social Work or closely related field.
Licensed Social Worker preferred. Clinical supervision is provided for all pursuing licensure as a social worker along with a reimbursement program for professional testing and licensing fees.
Outgoing, self-directed, have great written and oral communication skills, and enjoy working with a team.
Comfortable advocating for individuals with various levels of disability in a variety of settings.
Travel is required.
