HEAD START CASE MANAGER LINCOLN COUNTY
Case Manager will work with Head Start families to establish goals for personal, educational and vocational growth; identify problems and needs affecting or inhibiting the realization of these goals; develop plans toward their achievement; and ensure effective and timely use and coordination of available services.
QUALIFICATIONS: Bachelor’s Degree in Social Work or related field required, LSW or MSW preferred.
Experience in social service case management and supportive counseling, preferably working with low-income families.
Knowledge of family systems and ability to develop rapport with and establish trust and respect with families and staff.
Knowledge and understanding of all pertinent laws and regulations, including child abuse and neglect, duty to warn, etc.
Knowledge of state and local social services and resources available to low-income families.
Ability to work independently with minimal supervision. Ability to organize paperwork to meet deadlines. Ability to work as part of an integrated service team.
Must have valid driver’s license, adequate insurance, and/or reliable transportation. Must conduct all business with clients in a helpful and professional manner. Must be able to maintain confidentiality regarding any access to client or agency information. RESPONSIBILITIES:
*Conducts comprehensive family intake, evaluation, and assessment to determine areas of risk for families.
*Completes Family Security Assessment on each family within time frames listed in the Family Services plan. *Works with families and/or individuals to identify services and establish a time-specific plan to assist family members to work towards personal, educational, and vocational goals. Helps families and individuals to obtain community services, and advocating for clients where appropriate.
*Records and maintains comprehensive case management data within uniform formats, as required, that record client problems, goals, plans, program utilization, gaps in service, and outcomes for program and service system evaluation.
*Adheres to Social Work Code of Ethics.
*Maintains confidentiality regarding all client and agency business conducted at MMCA. Other duties as assigned.
